The population Health Institute, in collaboration with the Wisconsin Department of Health Services, is conducting an online survey about perceptions, awareness, and use of substances in Wisconsin. Anyone who is over 18 and lives in Wisconsin is encouraged to take this anonymous survey. Participants will have the opportunity to enter into a drawing for a cash prize of $200.
